Within the Good Faith Estimate tool, there are a couple of places where you can add additional notes that will show up on the printed or electronic version of the estimate.
Per Item
While adding a new charge to the estimate:
- If notes or additional information is needed for a specific charge, they can be added at the bottom of the pop-up box when you add the charge to the estimate.
After the charge has been added to the estimate:

Click on the edit pencil icon. The screen will open so you can add any notes that are needed.
Additional overall notes
If there are notes that are needed which are overall or provider/facility related, they can be added to the Additional Notes section.
