Where does the Reason for Referral information come from?

When you send a referral, there is a Reason for Referral section at the bottom of the referral that shows the reason for referral, who referred the patient, and who the patient was referred to. 

The reason will populate the information entered in the reason field. 

The referred by contact address will populate in two ways:

1. The office associated with the appointment with the last locked clinical note will be always be used as the address in the referred by contact.
2. If there are no locked clinical notes, the office address associated with the most recent or future appointment will be used. 

The referred to contact will populate with the information you have saved for that contact.
