Objective
Record all of the following demographics:
- Preferred language
- Gender
- Race
- Ethnicity
- Date of Birth

For all unique patients, you must record a subset of demographics using drchrono.
Measure
More than 50 percent of all unique patients seen by the EP have demographics recorded as structured data.
DENOMINATOR: Number of unique patients seen by the EP during the EHR reporting period.
NUMERATOR: Number of patients in the denominator who have all the elements of demographics (or a specific exclusion if the patient declined to provide one or more elements or if recording an element is contrary to state law) recorded as structured data.
The resulting percentage (Numerator ÷ Denominator) must be more than 50 percent in order for an EP to meet this measure.
Exclusions
There are no exclusions.

Tip: If you use OnPatient or the Check-in app, you can have patients fill this information out themselves before they are even in the exam room!
How to (website)
Step 1: Search for the patient using the spotlight search, or Patient list.
Step 2: Enter in the Patients demographics from the Demographics section.
Step 3: Scroll to the bottom of the page and click Save Demographics.
How to (iPad)
Step 1: Click on the patients name and then Patient Chart
Step 2: Navigate to the Patient tab >click on Patient Information > click on the pencil icon next to Name & Sex, and Background to edit each section.
