Core 5: Record Smoking Status


Record smoking status for patients 13 years old or older. 


More than 80% of all unique patients 13 years old or older seen by the eligible professional have smoking status recorded as structured data. 

  • DENOMINATOR: Number of unique patients age 13 or older seen by the provider during the EHR reporting period. 
  • NUMERATOR: The number of patients in the denominator with smoking status recorded as structured data.
  • THRESHOLD: The resulting percentage must be more the 80% in order for an eligible professional to meet this measure.


Any eligible professional that neither sees nor admits any patients 13 years old or older.

How to (Website)

Step 1.) Click on the patients appointment in the schedule

Step 2: Use the Vitals section to enter in a smoking status

Step 3a: Once you have selected the correct smoking status click on "Save & Close" in the appointment tab. 

Step 3b: Alternatively, you can use the MU Helper tab, which is also in the appointment screen to record the patients smoking status. 

Step 3c.) Once you have selected that patients smoking status under the MU Helper tab click on "Save MU Data"

Lastly, you are able to enter in a patients smoking status from the demographic section of their patient chart under the "Smoking Status" tab. Be sure to click "Save Demographics" once you have entered their status in.

How to (iPad)

Step 1: Select the patient and click on "Patient Chart" 

Step 2: At the top right of the page next to the chief complaint field click on "Vitals" 

Step 3: Choose the status from the "Smoking Status" drop down of the Vitals pop-up and click "Save"
