Core 14: Medication Reconciliation


The eligible provider who receives a patient from another setting of care or provider of care or believes an encounter is relevant should perform medication reconciliation


The eligible provider who preforms medication reconciliation for more than 50% of transitions of care in which the patient is transitioned into the care of the eligible provider. 

  • DENOMINATOR: Number of transitions of care during the EHR reporting period for which the eligible provider was receiving party of the transition. 
  • NUMERATOR: The number of transitions of care in the denominator where medication reconciliation was performed. 
  • THRESHOLD: The resulting percentage must be more than 50% in order for an eligible provider to meet this measure. 


Any eligible provider who was not the recipient of any transitions of care during the EHR reporting period.

How to (Website)

Step 1.) Create an appointment for the patient, if the patient is new click the + next to the Patient field. 

Step 2.) Search for the patient either by using the spotlight search, or through your patient list. 

Step 3.) Click on "Clinical Dashboard > Scroll to the bottom of the page and choose C-CDA from the drop down menu > Choose the C-CDA file you received for the Transition of Care > Click on "Preview File" 

Step 4.) Enter in a description and the date of import > click "Finalize File" 

Step 5.) After the import has finished choose "Medication List" from the side menu > Click "Merge" on the right side of each new medication from the C-CDA import.
