Menu 5: Patient-specific Education Resources


Use certified EHR technology to identify patient-specific education resources and provide those resources to the patient if appropriate. 


More than 10 percent of all unique patients seen by the EP are provided patient-specific education resources. 

DENOMINATOR: Number of unique patients seen by the EP during the EHR reporting period. 

NUMERATOR: Number of patients in the denominator who are provided patient-specific education resources. 

The resulting percentage (Numerator ÷ Denominator) must be more than 10 percent in order for an EP to meet this measure.

How to (Website)

Step 1: Search for the patient using the spotlight search, or by going through your patent list. 

Step 2: From the the patients file click on Education Resources 

Please Note: By clicking on Education Resources you are verifying you have provided education materials and have met this measure. The best workflow for this would be going into the patients file from the appointment screen. This way you will not have to search for the patient.
