Menu 6: Medication Reconciliation


The EP who receives a patient from another setting of care or provider of care or believes an encounter is relevant should preform medication reconciliation. 


The EP performs medication reconciliation for more than 50 percent of transitions of care in which the patient is transitioned into the care of the EP

DENOMINATOR: Number of transitions of care during the EHR reporting period for which the EP was the receiving party of the transition.

NUMERATOR: Number of transitions of care in the denominator where medication reconciliation was performed.

The resulting percentage (Numerator ÷ Denominator) must be more than 50 percent in order for an EP to meet this measure. 


An EP who was not the recipient of any transitions of care during the EHR reporting period.

How to (Website)

Step 1: Create a Transition of Care appointment for the patient. 

Step 2: Search for the patient either by using the spotlight search, or through your patient list.

Step 3: From the Clinical Dashboard section of the patients chart scroll to the bottom of the page and choose C-CDA from the drop down menu. Then choose the C-CDA you received for the Transition of Care and click on Preview File.

Step 4: Enter in the description and the date of import and then click Finalize File.


Step 5: After the import has finished, from the side menu choose Medication List. After, on the right side of the new medications click Merge to complete the process.
