Description: Percentage of patients 18 years and older who were screened for tobacco use one or more times within 24 months AND who received cessation counseling intervention if identified as a tobacco user.

Denominator Logic

Patient was >= 18 years old on or before the start of measurement period.


AND

        OR Patient had two or more appointment scheduled during measurement period, with CPT code in [90791, 90792, 90832, 90834, 90837, 90845, 92002, 92004, 92012, 92014, 96150, 96151, 96152, 97003, 97004, 99201, 99202, 99203, 99204, 99205, 99212, 99213, 99214, 99215, 99341] entered.
        OR Patient had one or more appointment scheduled during measurement period, with CPT code in [92521, 92522, 92523, 92524, 92540, 92557, 92625, 99385, 99386, 99387, 99395, 99396, 99397, 99401, 99402, 99403, 99404, 99411, 99412, 99420, 99429] entered.

Numerator Logic

OR Patient had smoking status "Former smoker" or "Never smoker" recorded before or during measurement period.
 

OR

        OR Patient had smoking status other that "Former smoker" or "Never smoker" recorded before or during measurement period.
 And patient had medication Tobacco Use Cessation Pharmacotherapy (CQM ValueSet: 2.16.840.1.113883.3.526.3.1190) ordered <= 24 months starts before or during measurement period

        OR Patient had one or more appointment scheduled <= 24 months starts before or during measurement period, with CPT code in [99406, 99407] or HCPCS code in [G0375, G0376, G8402, G9016, S4995, S9075, S9453] entered.

Denominator Exception Logic

Patient had active patient problem Limited Life Expectancy (CQM ValueSet: 2.16.840.1.113883.3.526.3.1259) recorded during measurement period .

How To (Website)

Step 1: Access patient chart, and click Smoking Status

 

Step 2: Click “Add Record” if a smoking record doesn’t currently exist for the patient, and click “Save Demographics” to save:

 

Step 3: Once the record is added, if you want to modify the smoking status for the patient, you can click “New Smoking Status” and edit the smoking status for the patient as pictured below. To save, click “Create”.
