CQM Measure CMS69v5 Preventive Care and Screening: Body Mass Index (BMI) Screening and Follow-Up Plan
NQS Domain: Patient Safety
Description: Percentage of patients aged 18 years and older with a BMI documented during the current encounter or during the previous six months AND with a BMI outside of normal parameters, a follow-up plan is documented during the encounter or during the previous six months of the current encounter.
Normal Parameters: Age 18 years and older BMI => 18.5 and < 25 kg/m2
Patient was >= 18 years old on or before the start of measurement period.
AND Patient had one or more appointment scheduled during measurement period, with CPT code [90791, 90792, 90832, 90834, 90837, 96150, 96151, 96152, 97001, 97003, 97802, 97803, 98960, 99201, 99202, 99203, 99204, 99205, 99212, 99213, 99214, 99215, 99385, 99386, 99387, 99395, 99396, 99397] or HCPCS code [G0101, G0108, G0270, G0271, G0402, G0438, G0439, G0447] entered.
OR Patient had active patient problem Pregnancy Dx (CQM ValueSet: 2.16.840.1.113883.3.600.1.1623) recorded during measurement period.
OR Patient had Intervention Palliative Care (CQM ValueSet: 2.16.840.1.113883.3.600.1.1579) ordered during measurement period.
OR: Patient most recent BMI recorded <= 6 months starts before or during measurement period had result >= 18.5 kg/m2 and result < 25 kg/m2
OR: Patient had Intervention Above Normal Follow-up (CQM ValueSet: 2.16.840.1.113883.3.600.1.1525) or Intervention Below Normal Follow up (CQM ValueSet: 2.16.840.1.113883.3.600.1.1528) or Intervention Referrals where weight assessment may occur (CQM ValueSet: 2.16.840.1.113883.3.600.1.1527) ordered <= 180 days before the BMI recorded date and during measurement period.
OR: Patient had Medication Above Normal Medications (CQM ValueSet: 2.16.840.1.113883.3.600.1.1498) or Medication Below Normal Medications (CQM ValueSet: 2.16.840.1.113883.3.600.1.1499) ordered <= 180 days before the BMI recorded date and during measurement period.

Step 1: Access “Schedule Appointment” area of patient record
Step 2: Click on the “Vitals” tab
Step 3: Enter BMI data for the patient in the BMI field as pictured below
Step 4: To enter a CPT or HCPCS code, access a patient's appointment (remember the appointment must be within the reporting period), and enter one of the applicable codes as shown below:
