Stage 1 - Objective 8: Patient Electronic Access (VDT)

Measure 1: More than 50 percent of all unique patients seen by the EP during the EHR reporting period are provided timely access (within four days) to view online, download, and transmit to a third party their health information subject to the EP's discretion to withhold certain information.

 

Measure 2: For an EHR reporting period in 2015, at least one patient seen by the EP during the EHR reporting period (or patient-authorized representative) views, downloads or transmits to a third party his or her health information during the EHR reporting period.

 

Exclusion: Providers may claim an exclusion for the second measure if for an EHR reporting period in 2015 they were scheduled to demonstrate Stage 1, which does not have an equivalent measure.

How to Measure 1

Step 1.) Search for the patient for whom you would like to enable access. You can search by either using the spotlight search bar, or through your patient list. 

Step 2.) When in the patients file click on "Patient Access" in the side menu, and then on "Enable Access" 

1 or more of your patients MUST login to their onpatient account. 

 

How to Measure 2

There are two ways you can easily view, save, or print your clinical summary through your onpatient portal.  

Viewing your clinical summary directly on the website:

  1. Login to your onpatient account.
  2. Under my "Health Profile" select "Entire Health Profile."

       3.  On the left hand side of the page select "Complete Profile."

      4.   Your full clinical summary will now appear on a new tab for you to view. 

 

Downloading your clinical summary as a pdf file:

  1. Login to your onpatient account.
  2. Under my "Health Profile" select "Entire Health Profile."
  3. On the left hand side of the page select "PDF file." 

  4. Next you will select "Download" on the right hand side.  Your clinical summary will download as a pdf file and you can now save or print the document.
