OR Patient had outbound referral sent during measurement period (via Message Center)
OR Patient had Intervention Referral (CQM ValueSet:2.16.840.1.113883.3.464.1003.101.12.1046) recorded during measurement period.
OR Patient had Patient Communication Consultant Report (CQM ValueSet: 2.16.840.1.113883.3.464.1003.121.12.1006) reported after referral sent
OR Patient had Patient Communication Consultant Report (CQM ValueSet: 2.16.840.1.113883.3.464.1003.121.12.1006) reported Intervention Referral ordered during measurement period.
