If you find that a billed line item was incorrect, the steps below will show you how to delete the identified code and rebill (if desired) with a replacement code.
- Navigate to Billing > Live Claims Feed
- Pull up the patient's account via their name, chart id, or claim number. Click on the date of service to enter the appointment specifics.
- Scroll down to where the charges are located
- For our scenario, let's assume that CPT 99203 was billed in error and CPT code 99213 should have been billed.

- If the charge has not been billed, considered, or paid by the payer:
- You can delete the code, enter the correct code, and re/bill the claim.
- If desired, you can also enter a note in box 19 to alert/explain the rebill.
- If by chance the payer has issued payment that has not been received at the time the rebill occurs (and you've deleted the code), any payment will post to the Unmatched ERA section if received via ERA.

- If the charge has not been billed, considered, or paid by the payer:

- If the charge has been paid on, we recommend:
- Deleting any adjustments taken by the payer (CO45 or other)
- Deleting any patient responsibility entires (deductible/copay/non covered charge/etc)
- Remove/Zero out the charge for the code that is being removed from the bill, but leave the code on the appointment.
- Adding the correct code
- Adding a note in box 19 to alert/explain the rebill.

This is how it will look after you remove any posted adjustments and patient responsibility entries. The negative amount in the Insurance Balance column is correct. It is showing that the payer (Aetna in this example) has paid $80.00 towards this charge and because of the code deletion, is now an overpayment.
Once you add the correct code, it will look like this:

- The red arrow shows where you have removed the charge amount from the code billed in error.
- The blue arrow shows the charge for the correct code that you've added.

- The red arrow shows where the payer's overpayment on the code sent in error is notated.
- The blue arrow shows the charge for the correct code that you've added.
Once all has been entered, select status "Bill Insurance" and click "Verify & Save". The claim will be sent to the clearinghouse during the next scheduled run.
