Box 69 - Where do I add the admitting diagnosis on an institutional claim?

   The admitting diagnosis must print in box 69 on the UB04 institutional claim form.  DrChrono makes it easy to add to your patient's claim.

• Navigate to Billing > Live Claims Feed > inside patient's appointment > Line items transactions section
• The admitting diagnosis code is added to the "Adm" box highlighted below.  The diagnosis can be added by code or looked up using text.   

• The diagnosis added to this box will be listed as the admitting diagnosis code in box 69 of the UB04 form.
