If you are a patient looking for your CCDA file, you can access it by first logging into your OnPatient account. Once logged in, select Health Profile at the top right and then select Complete Profile on the left and the Patient Health Summary will populate (see screenshot below).
You have the option to download the CCDA file as a PDF (select the Download PDF button) or XML file (select the Download C-CDA button).
You can also choose to send your C-CDA file if you'd like. Input your email in the Send CCDA to field, check the Encrypt box if you’d like to encrypt your file, and then hit the Send button.
