DrChrono Payments: Stripe business verification

  While opening an account to process patient payments, Stripe may need additional documents for business verification.  Here is a list of documents they deem acceptable for providers in the United States. 


Below is a listing of documents that Stripe can accept as proof of identity, address, and entity. Some forms of documentation require scans of both the front and back of the document. For these, use the document_back parameter to provide the back of the document. Unless explicitly noted, only a scan of the front of the document is required.

Acceptable forms of identification:

  • Passport
  • Passport card
  • Driver license
  • State issued ID card
  • Resident permit ID / U.S. Green Card
  • Border crossing card
  • Child ID card
  • NYC card
  • U.S. visa card

Acceptable forms of legal entity verification:

  • IRS Letter 147C
  • IRS SS-4 confirmation letter
