Clinical Notes in the C-CDA with Clinical Form Management

With Clinical Form Management in DrChrono, you can choose the type of clinical note you are creating.  Each form type has a LOINC code associated with it.  When the C-CDA is generated, the LOINC code associated with the form type will be included.  

To build a form navigate to to Clinical >Form Tools> Clinical Form Management. 

Select new form 

Click the gear icon.

Click Edit Form Info.

Name the form and select the Form Type.

Click Update to save the form type.

Complete the form using the form-building tools.

The forms will appear under the additional tab for you to use with your clinical note.

Once you have signed and locked the clinical note, the contents of the note will be available in the C-CDA.  

Clinical notes will only appear on the CCDA file based on the form type selected for the form. Form types MUST be associated with a form for the clinical note generated text to appear in the CCDA file. 

Persistent forms will no longer automatically populate within the "Assessment" section within the CCDA file. A form type must be set for the form's generated text within the rendered clinical note PDF to appear within the CCDA file.

In the XML file for the C-CDA, the LOINC code associated with the form type is included.
