Beginning in February 2026, we will start retiring the legacy Form Builder as part of the transition to Clinical Form Management. For more details and answers to common questions, please refer to our FAQ article.
You can recover templates (forms) that you have archived (deleted) from the form builder to use again.
1. Go to Clinical > Archive.
2. Click on Restore beside the respective template. The selected form will return to the "Additional" section of your Form Builder and be available for use.
