How do I disconnect my OnPatient account?

If you need to disconnect your OnPatient account from your provider, you can easily complete this process through your OnPatient account.

1. After logging in, select the Doctors tab and click Disconnect.

2. A window will open to confirm, click OK.

3. After disconnecting, there will no longer be any information connecting you to the practice.
