UB04 Box 15 - What is a Point of Origin Code and how do I include it on an institutional claim?

A point of origin code discloses to the payer the source or method of the patient's referral for admission. The point of origin code is similar to a "place of service" code on a professional claim/HCFA-1500 form.


To add it to an institutional claim/UB04/HCFA 1450 institutional form:

  1. Navigate to Billing > Live Claims Feed > Inside patient's appointment > right side of the screen > Info tab


The options under the drop-down include:

1- Non-healthcare facility

2- Clinic

4- Transfer from a hospital

5- Transfer from an SNF or ICF

6- Transfer from another healthcare facility

7- Emergency room

8- Court/law enforcement

9- Information not available

B- Transfer from another home health agency

C- Readmission to the same home health agency

D- Transfer from one distinct unit of the hospital to another distinct unit of the hospital resulting in a separate claim to the payer

E- Transfer from an ambulatory surgery center

F- Transfer from hospice and is under a hospice plan of care or enrolled in a hospice program
