Using standardized codes helps ensure clinical consistency, supports interoperability across systems, and improves the clarity of care documentation. This article reviews how to link SNOMED and LOINC codes to care plan goals and interventions during care plan creation or editing.

Link SNOMED and LOINC Values with Care Plan Goals

  • Start creating or editing a care plan goal.
  • In the free text field, begin typing the description of the goal.
  • As you type, SNOMED and LOINC code suggestions automatically appear below the field, similar to macros.
  • Review the suggestions and click on the appropriate SNOMED or LOINC code.
  • The selected code and its associated standardized text will populate the text field automatically.

This ensures that the care plan goal uses standardized clinical language and is properly coded for interoperability.

Link SNOMED and LOINC Values with Care Plan Interventions

  • Start creating or editing a care plan intervention under the relevant objective.
  • In the intervention free text field, begin typing the intervention details.
  • As you type, a list of SNOMED and LOINC code suggestions appears below the field.
  • Click on the appropriate suggestion to link the SNOMED or LOINC code to the intervention.
  • The field will update with the selected standardized text associated with the chosen code.

Using these codes helps maintain consistency, accuracy, and alignment with clinical documentation standards.
