This article explains what to do when information appears in your complete clinical note but the corresponding field appears empty or unselected in the edit section.
What You Might See
You may notice that:
- Text appears in your complete clinical note (like "Adrenal Support: 1 cap in the AM/ 1 cap in the afternoon (no later than 3pm)")
- But when you check the same field in the edit section, nothing is selected
- The field appears empty even though the information shows up in the final note
Why This Happens
This occurs when a field option has been removed from the Form Builder after it was originally selected in a clinical note. The system remembers the old selection, but that option no longer exists in the current form.
How to Identify the Problem
Look for these warning signs:
- Red outline around the field value with a warning message that says:
"Warning: '[Field value]' has been removed. Please select a different value or add this value back to this field in the Form Builder."
- Error message when trying to sign and lock the note that lists the affected fields
How to Fix It
You have two options to resolve this:
Option 1: Delete the Old Information
- Go to the Edit Section of your clinical note
- Find the field with the red outline or warning
- Delete the outdated information from that field
- Save your changes
Option 2: Add the Value Back in Form Builder
- Access the Form Builder
- Find the relevant form field
- Add the missing value back to the field options
- Return to your clinical note and verify the field now displays correctly
- You cannot sign and lock a note that has invalid field values
- The system will show you exactly which fields need to be corrected before you can proceed
- This ensures your clinical notes always contain accurate, valid information
