This article provides an overview for managing objectives and interventions within a patient care plan. Objectives allow you to define measurable outcomes tied to each care plan goal, while interventions specify the actions that will help achieve those objectives. Together, these components create a structured and actionable care plan that supports ongoing patient care and coordination across the care team.

Adding an Observation (Objective) to a Patient Care Plan
Each care plan Goal can include multiple Objectives. This below process allows you to track specific observations or goals linked to the patient’s care plan.
- Click the + Objective button within the care plan.
- A new objective entry appears in the care plan card.
- Enter the Objective details in the free text field.
- Select a Target Date by clicking on the date picker and choosing the desired date.
- Choose the Status from the dropdown menu. Available options include:
- Not started
- In Progress
- Partially Complete
- Complete
- Patient Refused
- Held
- Normal
- Nullified
- Obsolete
- Suspended
- To save the objective, click Save. This applies your changes and adds the objective to the care plan.
- To discard the objective, click Cancel. No changes are saved, and the objective is not added.
Edit an Objective
If an objective already exists for the patient, you can edit it by following these steps:
- Locate the objective in the care plan.
- Click the pencil icon next to the objective to open the edit drawer.
- Update the objective fields as needed within the drawer.
This allows you to modify the objective details while maintaining the current record.
Adding an Intervention to a Patient Care Plan Objective
This below process helps specify planned actions related to the patient’s care goals.
When you remove a goal, objective, or intervention, a confirmation modal appears before the action is completed. This step helps prevent accidental deletions by requiring manual confirmation.
- Review the content displayed in the confirmation modal to ensure it is the correct item.
- Click the Delete button in the modal to confirm and complete the removal.
