Smoking Status Options
A single-select dropdown is shown with the following options (the system stores the description and code):

Document Smoking Status
Users can:
- Select a smoking status from the dropdown
- Enter Start and End Date
Auto-Populate Code Attachment
Upon selection:
- The SNOMED code is automatically populated in the Code attachment section (not editable)
- Users can add other associated codes manually
- SNOMED codes are automatically populated upon selection and cannot be edited
The Customized Note section keeps its current functionality, allowing the user to edit.
CCDA Output
The data is displayed correctly in the CCDA PDF generator using the appropriate SNOMED codes.
Backfill
Once smoking status is added to the form, the system will backfill the smoking status field in Demographics > Smoking status section.
