You can access your health information in the OnPatient portal after appointments are completed.
Access your health record
Select Health Record. The Problems section opens by default.

Health section overview
| Section | Description |
|---|---|
| Problems | Diagnosed conditions on your chart, showing the problem name, the diagnosing provider, and the date of diagnosis. |
| Allergies | Recorded allergies (drug and non-drug), along with severity, the reaction experienced, and the provider who documented it. |
| Medications | Shows current prescriptions including the medication name, dosage instructions, the prescribing provider, and the date they were added to your chart. |
| Preferred Pharmacies | Add and manage pharmacies where you'd like your prescriptions sent. |
| Lab Results | Lab test results your provider has released. |
| Immunizations | Vaccines you've received, showing the immunization name, date given, the ordering provider, and who administered it. You can print a copy or save a PDF. |
| Vitals | Recorded vital signs such as temperature, pulse, blood pressure, respiratory rate, oxygen saturation, pain level, and smoking status, each with a timestamp. The most recent vitals show, but you can select one to view how it has trended over time. |
| Measurements | Physical measurements like height, weight, and BMI, along with pediatric-specific percentiles when applicable, each with a timestamp. The most recent measurements show, but you can select one to view how it has trended over time. |
| Education Documents | Any patient education materials your provider has shared with you. |
| Export | You can generate a patient health summary to share with another practice. You can preview a PDF version of the C-CDA file and email, download, or print the file. |
| Access Logs | Provides a log of any time your C-CDA has been accessed, including who accessed it, a timestamp, and what action was taken (view, download). |
