Clinical Notes in the C-CDA with Clinical Form Management

With Clinical Form Management in DrChrono, you can choose the type of clinical note you are creating.  Each form type has a LOINC code associated with it.  When the C-CDA is generated, the LOINC code associated with the form type will be included.  

Configuring a form type

To build a form navigate to:

  • Clinical > Clinical Form Management 
  • Select new form  
  • Click the settings gear icon

  • Click Edit Form Info.

Name the form and select the Form Type.

  • Click Update to save the form type.

  • Proceed with build the form using the form-building tools.

  Forms will appear under the "Additional" tab for use within your clinical note. Once the clinical note is signed and locked, its contents will be available in the C-CDA file; however, clinical notes will only appear in the C-CDA if the associated forms have an assigned form type.  Additionally, persistent forms will no longer automatically populate in the "Assessment" section of the C-CDA—only forms with an assigned form type will have their content reflected in the rendered clinical note PDF and included in the C-CDA.

In the XML file for the C-CDA, the LOINC code associated with the form type is included.
