ONC Update 2025: Patient Problem List

DrChrono has implemented critical updates to align with the requirements set forth by the Office of the National Coordinator for Health Information Technology (ONC) under the Health IT Certification Program’s HTI-1 final rule. A key focus of this initiative is ensuring that the Problem List aligns with the data standards specified in the United States Core Data for Interoperability (USCDI) Version 3 (v3).

    • Cognitive Status ( located in clinical dashboard> mental status)
Behavior for Problem Type Selection
  • Social Determinants of Health (SDOH)
    • The system will auto populate the Problem Type as SDOH if the provider enters a diagnoses that falls within the SDOH value set
    • when a user selects SDOH, a secondary dropdown will appear with the value sets
    • Users cannot manually assign SDOH as a problem type it must be determined based on the associated diagnoses
  • Functional Status, Disability, and Cognitive Status

    • These options will remain manually selectable 

    • There will be no automated logic
