(ONC Update) Health Concern Assessment


The Health Concern feature allows clinicians to flag relevant concerns during structured data entry, ensuring interoperability via CCDA using the correct LOINC code for Health Concerns (75310-3).

The following field types in the Form Builder support the ability to be marked as a Health Concern ( excluded: Numeric fields, Headers, Subheaders, Freedraw (sketch fields) :

User Workflow in Clinical Form Management

Each supported field includes a "Mark as Health Concern" toggle in field configuration.

When toggled on:

  • A Health Concern tag is associated with the field
  • A customized text format is generated for the Clinical Note, based on:
    • Field label
    • Value(s) selected or entered
    • Date if applicable

Clinical Note Behavior

The output of Health Concern–flagged fields appears in the Clinical Note using standard formatting:

  • Switches/Yes-No fields reflect the value (e.g., "Fall Risk: Yes")
  • Free text values show full user-entered content
  • Multi-select or checkbox values list all selected concerns inline

Customized Note Section

The Customized Note section:

  • Is editable by the user
  • Defaults to the requirements listed above in "Clinical Note Behavior"
  • If the note is not edited, this default format is used in downstream outputs

CCDA Output

Fields marked as Health Concerns:

  • Are included in the Health Concerns section of the CCDA PDF generator
  • Map to LOINC code 75310-3
  • Are displayed as structured entries with date (if captured)

Chart-Level Health Concern Designation

Problem List Integration: Clinicians can mark a Problem (active or resolved) as a Health Concern via an inline toggle or checkbox. This status is stored and rendered in both:

  • CCDA Health Concerns section
  • Patient Summary or Problems list (optional: badge or visual tag)

Allergies List Integration: Clinicians can mark an Allergy as a Health Concern using the same toggle behavior as above. Status reflects in CCDA and optionally in Allergy List UI.

Users can:

  • Use the date picker field to document the date
  • Select status field with the following options:
    • Active
    • Completed
